ABD’de her yıl yaklaşık 1.5 milyon kişi kalp krizi geçirmektedir. Kalp krizi geçirenlerin üçte bir kadarı ilk 20 günde, %3-%12’si de bir yıl içinde yaşamlarını yitirmektedir. Enfarktüsün nedeni kısaca kolesterolle ve yağla dolu plak tarzında lezyonların damarı tıkamasıdır.
Belirtiler
Kalp krizi ya da enfarktüs; kalbin koroner arterlerinde gerçekleşen bir bozukluk sonrası meydana gelen yetersizlik sonucu şiddetli göğüs ağrısıyla ortaya çıkan ve ölümle sonuçlanması olası fizyolojik durum. Dünyada en başta gelen ölüm sebeplerindendir.Bazı belirtileri bulunmakta ve dikkat edildiğinde hayat kurtarıcı olabilmektedir. Bunlardan başlıcaları;· Göğüs kafesinin orta bölgesinde birkaç dakikadan uzun süren baskı, sıkışma, ağırlık, huzursuzluk, Adrenalin deşarjı ve ölüm hissi.· Omuzlara, boyuna veya kollara yayılan göğüs ağrısı;· Aritmiler· Baş dönmesi, baygınlık, bayılma, bulantı, [[soğuk terlemeyle beraber göğüs kafesi şikayetleridir.
Tanı
Tanı öykü ve klinik bulgularla konur.
Tedavi
İlk tedavi önlemleri oksijen, nitrogliserin, düşük doz aspirin ya da ağrı tedavisi olabilir. Kalpteki ritim bozukluklarını giderici antiaritmik ilaçlar ve kalp kasındaki hasarın ilerlemesini önlemek için beta-blokerler verilebilir. Hasta krizden sonraki ilk birkaç saat içinde hastaneye yatırılırsa pıhtıları eritmek için trombolitik ilaçlar verilebilir. Ayrıca anjiyoplasti (daralan kalp damarlarının genişletilmesi) ya da koroner arter köprüleme (bypass) ameliyatı yapılabilir.

Hamile olup olmadığınızı en erken anlamanın yolu gebelik testi yaptırmaktır…
4 günlük döllenmiş yumurta, insan koryon gonadotropini hCG denilen bir hormon salgılamaya başlar. Bu hormon vücut suyunda vücudun dokularına yayılır. Başlangıçta kanda bulunabilir, kısa bir süre sonra ise idrarda teşhis edilebilir.
Gebelik testlerinin çoğu, kan örneğinden daha kolay alınan idrar örneği ile yapılır.
hCG nin varlığını tespit etmek için birçok idrar tahlili tipi kullanılır. Çoğu, hCG ile bir hCG antikoru arasındaki bir reaksiyona dayanır. İlk reaksiyonun gerçekleşip gerçekleşmediğini belirlemek için ikinci bir reaksiyona ihtiyaç vardır. Bu genellikle bir renk değişimidir.

İdrara dayalı gebelik testleri en sık olarak kullanılmaktadır, ama bazı durumlarda, daha kesin olduğu ve hamileliği daha erken teşhis edebildiği için bir kan testi de uygulanabilir. Dış gebelik yumurtanın fallop tüpünde ya da rahim dışında bir yerde gelişmeye başladığı gebelik durumunda, idrara dayalı gebelik testlerinin negatif sonuçlarına karşın, kadında tüm gebelik belirtileri görülebilir. Bunun nedeni, yumurta rahimde olmadığı zaman HCG düzeylerinin yüksek olmamasıdır. Ancak kan testi daha hassastır ve gebeliği teşhis edebilir. Dış gebelik potansiyel olarak yaşamı tehdit eden bir durum olduğu için, bu bilgi annenin hayatının kurtulmasına yardımcı olabilir.

GEBELİK TESTLERİ

İdrar testleri:Kanınızda bulunan B-HCG belli bir eşik seviyesine olaştığında idrara çıkmaya başlar ve hamileliğinizin ilerlemesiyle idrardaki bu seviye artar. İdrarda yapılan bu testlerin amacı, B-HCG nin varlığının ya da yokluğunun saptanmasına dayanır. Çeşitli testlerin hassasiyeti arasındaki farklılıklar idrardaki seviyeyi tanıyıp tanıyamamalarına dayanır. Eczanelerde ya da evlerde hazır test kitleri yardımıyla uygulanan idrarda gebelik testlerinin güvenilirliği üretici firma tarafından her ne kadar 99 olarak belirtilse de yapılan çalışmalar özellikle adet gecikmesinin 10 günden daha az olduğu durumlarda hata oranının yüksek olduğunu göstermektedir

Pek çok insan değişik nesne ve durumlardan değişen derecelerde korkar ama epidemiyolojik çalışmalara göre toplumun %5-10'unda bu korkular "fobi" tanısı alabilecek kadar şiddetlidir. Sosyal fobi, başkalarınca eleştirileceği, onların yanında rezil olacağı ya da utanç duyacağı durumlara düşeceği endişesiyle sosyal ortamlara girmekten çekinme, insanlarası iletişim kurma konusunda korku duymadır. Pek çok insan için özel sosyal durumlarda yaşanan stres duyguları bilinen bir durumdur. İnsanların çoğu bir topluluk önünde konuşmaları gerektiğinde anksiyete yaşarlar. Bu anksiyeteye çoğunlukla somatik semptomlar da eşlik eder: ellerin terlemesi, yüz kızarması v.b. İnsanların bir kısmı böyle durumlarda anksiyeteleriyle başa çıkamaz ve bu tür ortamlardan kaçınırlar. Sonuçta bu kaçınma davranışları yüzünden sosyal ve mesleki işlevsellikleri bozulur.Sosyal fobide kişinin başlıca korkusu başkalarının yanında küçük düşeceği, sıkıntı duyacağı ya da utanç duyacağı bir biçimde davranacağı korkusudur. Bu hastalar başkalarıyla etkileşimde bulunmalarını gerektiren ya da bir eylemi başkalarının yanında yerine getirmeleri gereken durumlardan korkarlar ve bunlardan olabildiğince kaçınmaya çalışırlar. Sık görülen sosyal fobiler, başkalarının önünde konuşma, yemek yeme ya da yazı yazma, genel tuvaletleri kullanma, görüşmelere ve her türlü toplantıya katılmadır. Bu hastalar ayrıca toplumsal durumlarda yaşadıkları anksiyeteyi diğer insanların anlayacağından ve gülünç duruma düşeceklerinden korkarlar. Sosyal fobide yaşanan anksiyete, uyarana bağlı olarak ortaya çıkar. Fobik durumlarla karşılaşılması için zorlandiğında ya da beklenmedik bir anda böyle bir durumla karşı karşıya geldiğinde kişi yoğun bir anksiyete yaşar, bunun yanı sıra çok çeşitli somatik semptomlar gösterir. Değişik anksiyete bozukluklarına değişik somatik semptomlar eşlik eder. Örneğin; panik ataklarında daha çok çarpıntı, göğüste ağrı ya da sıkışma hissi görülürken sosyal fobide daha çok yüz kızarması ve ağız kuruluğu görülür. Sosyal fobisi olan hastaların korkulan durumlara bağlı olarak gerçek panik atakları da ortaya çıkabilir. Sosyal fobisi olan kişiler, çeşitli toplumsal durumlarda olumsuz bir biçimde değerlendirileceklerine ilişkin büyük bir korku duyarlar. Korktukları durumlarla karşılaştıkları zamanlarda sıklıkla anksiyetenin somatik semptomlarını yaşarlar. Sosyal fobide korku duyulduğu sırada en sık gözlenen somatik semptomlar şunlardır: çarpıntı(%79), titreme(%75), kaslarda gerginlik(%64), midede burulma duygusu(%63), ağızda kuruma (%61), ateş basması ya da üşüme(%57), ve kafada basınç duygusu ya da baş ağrısı.Sosyal fobi için tanı ölçütlerini karşılayabilmesi için toplumsal durumlarda korkmanın bir sonucu olarak, anksiyete ortaya çıkmış olmalıdır. Bazı kişilerin korkuları özgül toplumsal olaylarla sınırlı olmasına rağmen bazılarınınki hemen bütün toplumsal durumlarda görülebilir. En sık görülen sosyal fobik durumlar şunlardır: birisiyle tanıştırılma, konum olarak kendisinden daha yukarıda bulunan kişilerle karşılaşma, telefon etme, konuk kabul etme, birşeyler yaparken başkaları tarafından izlenme, başkalarının kendine "takılması", tanıdıklarıyla yemek yeme, başkalarının yanında yazı yazma ve toplum önünde konuşma.Sosyal fobinin bilişsel yanı, sıklıkla yakın gözlem ile anlaşılabilecektir. Bu, sosyal fobiğin kendi davranışını ve bunun diğerleri üzerindeki etkisini yargılaması ve değerlendirmesiyle ilgilidir. Arkowitz'e göre beş ana bilişsel mekanizma ve düşünce şekli vardır :1. Sosyal fobikler sosyal ilişkilere olumsuz yüklemeler yaparlar. Örneğin; "bakın ben herşeyi nasıl berbat ediyorum." ya da "benden ne köy olur ne de kasaba." gibi düşünceleri vardır.2. Sosyal fobiklerin kendi sosyal davranışlarını abartılı, olumsuz düzeyde aşağılama eğilimleri vardır.3. Yukardakilerle ilgili olarak, sosyal fobikler kendi davranışlarına yönelik abartılı negatif yüklemeler yaparlar ve genellikle diğerlerinin davranışlarını olumlu görürler. 4. Kendileri için oldukça seçicidirler, hoş ya da olumlu durum ya da olaylar, önemsiz kabul edilip bir kenara konur, bunun yanı sıra yetersiz, doyumsuz olaylar anımsanır ve uzun süre üzerinde durulur.5. Son olarak sosyal ilişkilerde hoş olan durumlarda kendileri dışında neden ararlar(dışsal yüklemleme), ancak hayal kırıklığı yaratan durumların nedenlerini kendilerinde ararlar.Sosyal fobi tanısı son dönemlerde genel kabul görmüştür. Sosyal fobi DSM-III'ten(1980) beri anksiyete bozuklukları içinde sınıflandırılmaktadır. DSM-III'te sosyal fobi "özel sosyal durumlardan ısrarlı korku, ortamdan kaçınma isteği ve bireyin anksiyeteyi temelde gerçek dışı olarak değerlendirmesi" biçiminde tanımlanmıştı. DSM-III'e göre birden fazla durumdan korkmak, sosyal fobi değildi; bu durum daha çok "çekingen kişilik bozukluğu" diye sınıflandırılıyordu. DSM-III-R'de ise bu değişmiş, çeşitli sosyal durumlardan korkan hastalar için "yaygın tip" şeklinde bir alt sınıf tanımlanmıştır.DSM-IV'e Göre Sosyal Fobi Tanı Kriterleri: Tanımadık insanlarla karşılaştığı ya da başkalarının gözünün üzerinde olabileceği, bir ya da birden fazla toplumsal ya da bir eylemi gerçekleştirdiği durumdan, belirgin ve sürekli bir korku duyma: kişi küçük duruma düşeceği ya da utanç duyacağı bir biçimde davranacağından korkar(ya da anksiyete belirtileri gösterir). Not: Çocuklarda, tanıdık kişilerle yaşına uygun toplumsal ilişkilere girebilme becerisi olmalı ve anksiyete, sadece erişkinlerle olan etkileşimlerinde değil, yaşıtlarıyla karşılaştığı ortamlarda da ortaya çıkmalıdır. A. Korkulan sosyal durumla karşılaşma, hemen her zaman anksiyete yaratır ki, bu anksiyete, duruma bağlı olarak, panik atak biçimini alabilir. Not: Çocuklarda anksiyete, ağlama, huysuzluk gösterme, donakalma ya da yabancı insanların olduğu toplumsal durumlardan uzak durma olarak görülebilir. B. Kişi, korkusunun aşırı ya da anlamsız olduğunun ayırdındadır. Not: Çocuklarda bu özellik olmayabilir. C. Korku ya da kaçınma davranışı, bir maddenin ya da genel tıbbi bir durumun doğrudan fizyolojik etkilerine bağlı değildir ve başka bir mental bozukluk(agorafobiyle birlikte ya da agorafobi olmadan panik bozukluğu, beden dismorfik bozukluğu, yaygın gelişimsel bozukluk ya da şizoid kişilik bozukluğu gibi) ile daha iyi açıklanamaz.G. Ayrıcı TanıPanik Bozukluk: Agorafobi ile birlikte panik bozukluğu; beklenmedik biçimde panik ataklarının ortaya çıkması, daha sonra da panik ataklarını ortaya çıkartabileceği düşünülen birçok durumdan kaçınılması ile karakterizedir. Tek korku, panik atağı olduğu sırada görülmek ise sosyal fobi tanısı konulamaz. Hem panik atakları hem de toplumsal kaçınması olan hastalar, bazen tanı sorunu yaratabilirler. Sosyal fobi, panik atakları olmadan toplumsal durumlardan kaçınma ile belirlidir. Panik atakları ortaya çıkarsa bu atakların sadece toplum önünde olup olmadığı araştırılmalıdır. Agorafobi: Panik ataklar olmadan gelişen agorafobide korkular kişi, başkalarının yanında olsun ya da olmasın belirli durumları kapsar. Örneğin evde tek başına kalma, tek başına dışarı çıkamama, yolculuk yapamama v.b. Oysa sosyal fobisi olan kişinin korkuları, başka insanların yanında olduğu durumlarla ilgilidir. Şizoid Kişilik Bozukluğu: Şizoid kişilik bozukluğunda kişi, diğerleriyle ilgilenme isteğinden yoksun olduğu için toplumsal durumlardan uzak durur. Oysa sosyal fobisi olan kişilerin toplumsal ilişkilere ve olaylara katılma istekleri vardır.Çekingen Kişilik Bozukluğu: Çekingen kişilik bozukluğunun sosyal fobiyle ortak bir takım özellikleri vardır ve çoğu zaman yaygın sosyal fobiyle ayırıcı tanı yapmak imkansızdır. Ancak sosyal fobinin belirgin bir başlangıç tarihi saptanabiliyorsa ve korkulan durum sayısı daha sınırlıysa, ayırıcı tanı daha kolaylıkla yapılabilir. Depresyon: Depresyonda hastanın yaşadığı çökkün duygudurum, isteksizlik nedeniyle hasta toplumsal olaylardan uzak durur. Sosyal fobilerde ise belirli bir korku vardır.Dismorfofobi: Bu bozuklukta kişi, fiziksel bir kusuru olduğu inancındadır. Bu nedenle sosyal ilişkilerden kaçınır. Ancak sosyal fobik hastaların aksine bu düşüncelerinin ve yaşadıkları anksiyetenin mantıksiz olduğunu düşünmezler.Alkol Bağımlılığı: Diğer anksiyete bozukluklarında da olduğu gibi sosyal fobisi olan kişilerde de, alkol kötüye kullanımı sık görülür. Ancak alkol kullanımının birincil mi, ikincil mi olduğunun araştırılması gerekir.EpidemiyolojiÖnceleri sosyal fobinin toplumun %1'inden azını etkilediği düşünülmekteydi, ancak yapılan epidemiyolojik çalışmalar sanıldığından daha yüksek oranlarda toplumu etkilediğini göstermiştir. ECA çalışmasında sosyal fobi %1,9-3,4 oranlarında ortalama %2,4 oranında bulunmuştur. Komorbidite taramasında ise bu oran daha yüksek bulunmuştur. Sosyal fobi; madde kötüye kullanımı ve depresyondan sonra en sık görülen üçüncü psikiyatrik bozukluk olarak saptanmıştır. Altı aylık prevelansı 100 kişide 2-3 arasındadır. Epidemiyolojik çalışmalarda, kadınlarda daha sık görüldüğü saptanmışsa da, kliniğe tedavi için başvuranların çoğu erkektir. Daha çok 13-19 yaşlarında başlar.Sosyal fobi başladıktan sonra, genellikle başka bir psikiyatrik bozukluk da eklenmektedir. Yapılan çalışmalarda sadece %8-39 arasında komorbidite saptanmıştır. En sık; major depresyon, agorafobi, panik bozukluğu ve alkol ve madde kötüye kullanımı sosyal fobiyle birlikte bulunmaktadır. Etyoloji Bir görüşe göre, sosyal fobide biyolojik faktörler esastır ve hastalık aslında biyolojik olarak bir anksiyete bozukluğudur. Biyolojik Etkenler: Sosyal fobiyle ilgili ilk çalışmalar, toplum önünde konuşma modeliyle yapılmıştır. Toplum önünde konuşmada görülen terleme, çarpıntı, kızarma, tremor gibi semptomlar sosyal fobide de görülen semptomlardır ve adrenerjik sistem tarafından kontrol edilmektedir. Dimsdale ve Moss(1980), normal gönüllülerde de toplum önünde konuşmada adrenalin seviyelerinin yükseldiğini, ama konuşma devam ettikçe azaldığını saptamışlardır. 15 dakika içinde adrenalin seviyesi normale dönmektedir. Sein ve ark.(1992)'de yaptığı bir çalışmada sosyal fobiklerde yatar pozisyonda ve ayakta noradrenalin seviyelerinin kontrol grubuna göre daha yüksek olduğunu saptamışlardır. Nöroendokrin Çalışmalar: Yapılan çalışmalarda hipotalamik-pituiter-adrenal(HPA) eksende ve hipotalamik-pituiter-tiroid (HPT) eksende sosyal fobi için ayırıcı nitelikte değişiklikler saptanamamıştır. İdrarda serbest kortizol seviyelerine bakılmış ve sosyal fobisi olan kişilerle kontrol grubu arasında fark bulunmamıştır(Potts ve ark.,1991; Uhde ve Tancer, 1994). Ayrıca dekzametazon supresyon testinde de, iki grup arasında farklılık yoktur. Sosyal fobisi olan kişilerde T3, T4, serbest T4 ve TSH hormon seviyeleri de normal sınırlar içinde bulunmuştur. Fakat sosyal fobisi olan kişilere TRH verildikten sonra 1.dakikada ölçülen kan basıncı kontrol grubuna göre yüksektir(Tancer ve ark., 1990). Kimyasal Provokasyon Testleri: Adrenalinle yapılan bir çalışmada adrenalin infuzyonundan sonra 11 sosyal fobik hastadan sadece bir tanesinde anksiyete semptomlarında artış olmuştur: Bu hastada katekolamin seviyeleri başlangıca göre 47 kat artmıştır.Yohimbinle yapılan çalışmalarda, yohimbinin intravenöz uygulanmasından sonra 6 hastada sosyal anksiyete semptomlarında noradrenalin metaboliti olan metoksi-hidroksifenilglikol(MHPG) seviyesiyle paralel olarak artış görülmüştür. Sodyum laktat, karbondioksit ve kafein uygulamalarıyla yapılan çalışmalarda, panik bozuklukta, sodyum laktat, CO2 ve kafein, panik atakları provoke etmekte kullanılmıştır. Bir çalışmada sodyum laktatın etkileri 15 sosyal fobik hastayla 29 panik bozukluğu olan hasta grubu arasında karşılaştırılmıştır. Panik bozukluğu olan hastaların %48'inde panik atak gözlenirken sosyal fobik hastaların sadece %6.7'sinde panik atak gözlenmiştir. CO2'in etkilerini tartışmak çok daha zordur, çünkü yapılan çalışmada sadece üç sosyal fobik hasta, 9 panik bozukluğu olan hastayla karşılaştırılmıştır. %52lik CO2 inhalasyonuyla panik bozukluğu olan hastaların %39'unda anksiyete semptomları görülürken sosyal fobik hastaların hiçbirinde değişiklik gözlenmemiştir. Eğer inhale edilen karışım %7'lik CO2 olursa, üç sosyal fobik hastanın üçünde de anksiyete semptomları gözlenmiştir(Gorman ve ark., 1988). Kafein uygulaması çalışmalarında ise, hem sosyal fobiklerde, hem de panik bozukluğu olan hastalarda, panik atakları gözlenmiştir. Ancak sosyal fobik hastalar bu yeni semptomların sosyal fobi semptomlarından farklı olduğunu belirtmişlerdir(Tancer ve ark.,1991).Nörotransmiter Sistemleri: Sosyal fobide dopamin, serotonin ve noradrenalin olmak üzere üç ana nörotransmiter sınıfı incelenmiştir. a) Dopamin: Tancer(1993), sosyal fobi etyolojisinde dopaminin olası etkilerini incelemek için farelere L-dopa vermiş ve dopaminerjik fonksiyonu ölçmek için de prolaktin seviyelerini ölçmüştür. Tancer, sosyal fobiklerle kontrol grubu arasında bir fark saptayamamıştır. Johnson ve ark.(1994)'nın yaptığı bir çalışmada, sosyal fobisi de olan panik bozukluklu hastalarda, BOS'ta dopamin metaboliti olan homovanilik asit düzeyi kontrol grubuna göre düşük saptanmıştır. MAOI'leriyle yapılan tedavi çalışmaları, sosyal fobide dopamin sisteminin de rolü olduğunu desteklemektedir. Sosyal fobi semptomlarında, fenelzin, bir TCA olan imipraminden daha iyi sonuç vermiştir(Liebowitz ve ark., 1984). Bilindiği gibi MAOI'lerinin dopaminerjik aktiviteleri TCA'lardan daha fazladır. Mikkelsen ve ark.(1981), haloperidol ile tedavi edilen Tourette sendromlu bir olguda sosyal fobi ve kaçınma semptomlarının başladığını bildirmişlerdir. MAOI'lerinin dopamin üzerinde olduğu kadar serotonin üzerinde de etkileri vardır. Bu yüzden de etkilerini ayırdetmek kolay değildir. Komorbidite çalışmalarında sosyal fobisi olan kişilerin daha sonra parkinson hastalığına yakalanma risklerinin daha yüksek olduğunu saptanmıştır. MRI çalışmalarında da dopamin disfonksiyonunu destekleyen bazal ganglion anomalileri saptanmıştır. b) Serotonin: Sosyal fobili hastalarda, fenfluramin(5-HT-salgılatıcı ajan) ile yapılan çalışmalar, post-sinaptik 5-HT reseptör süpersensitivitesinin varlığını desteklemektedir (Tancer, 1993). Bu çalışmada, fenfluramin uygulamasından sonra prolaktin cevabında kontrol grubu ile arada fark bulunmazken kortizol cevabı çok yüksek bulunmuştur. Stein ve ark. (1995), serotonin aktivitesi göstergesi olan platelet [3H]-paroksetin bağlanma ölçümlerinde sosyal fobik hastalarla, kontrol grubu arasında fark bulamamışlardır. Serotonerjik ilaçlarla tedavi çalışmalarının sonuçları ise, daha ümit vericidir. Fluoksetinin sosyal fobi semptomlarında belirgin düzelme yaptığına dair çalışmalar vardır. 5-HT1A agonisti olan buspironun da(45 mg ve üstü dozlarda) sosyal anksiyete semptomlarında orta derecede bir düzelme oluşturduğu bildirilmiştir. MAOI'lerinin pozitif etkisi de serotenerjik aktiviteleriyle açıklanabilir. MAOI'leri, terminal otoreseptör sensitivitesini ve 5-HT yıkımını azaltarak ortamdaki 5-HT düzeyini arttırmaktadır. Ayrıca somatodendritik ateşleme hızları da düştüğü için, her ateşleme ünitesine düşen 5-HT oranı da artmaktadır. c) Noradrenalin: Noradrenerjik fonksiyonu ölçmek için klonidin uygulanmış ve büyüme hormonu cevabına bakılmıştır. Tancer ve ark.(1989) sosyal fobi ve panik bozukluğu olan hastalarda büyüme hormonu cevabının körelmiş olduğunu bildirmişlerdir, fakat bu sonuç, daha sonraki çalışmalar da doğrulanmamıştır. Görüntüleme Çalışmaları: Soyal fobi çalışmalarında görüntüleme tekniklerinin kullanılması çok daha yenidir. Sosyal fobik hastalarda bazal ganglion ölçümleri yapılmış ve bu hastalarda yaşlanmayla birlikte total putamen hacminde azalma olduğu saptanmıştır. Ayrıca kaudat, talamik ve subkortikal alanlarda kolin ve kreatinin seviyeleri düşük bulunmuştur. KomorbiditeYapılan epidemiyolojik çalışmalarda sosyal fobisi olan hastalarda en sık görülen komorbid durumların basit fobi(%59) ve agorafobi(%44,9) olduğu bulunmuştur. Ayrıca bu hastaların %19'unda alkol kötüye kullanımı ve %17'sinde majör depresyon olduğu saptanmıştır. Sıklıkla eşlik eden diğer durumlar; panik bozukluğu, obsesif kompulsif bozukluk ve yeme bozukluklarıdır. Çoğu olguda, sosyal fobi semptomlarının komorbid bozukluktan önce başladığı saptanmıştır, bu da, sosyal fobinin, diğer bozuklukların ortaya çıkışını kolaylaştırdığını düşündürmektedir. Sosyal fobiyi erken tedavi etmek, komorbid bozuklukların ortaya çıkışın engelleyecektir.Tedavi Sosyal fobinin tedavisinde hem psikoterapi hem de farmakoterapi uygulanır. Günümüzde yaygın olarak kognitif-davranışçı terapi gittikçe ağırlık kazanmaktadır. Farmakoteapiye kognitif-davranışçı terapi de eklendiği zaman, farmakoterapi kesildiğinde, rölapsın daha az olduğu saptanmıştır. Ayrıca, yapılan kontrollü çalışmalarda; farmakoterapiyle birlikte kognitif-davranışçı terapinin tek başına farmakoterapiden veya psikoterapiden daha başarılı olduğu görülmüştür(Gelernter ve ark.1991). Farmakoterapi : Sosyal fobinin farmakolojik tedavisinde diğer anksiyete bozuklukları kadar ilerleme kaydedilememiştir. İlk çalışmalarda alprazolam ve klonazepamın sosyal fobi semptomlarında iyileşme sağladığı bildirilmiştir(Lydiard ve ark. 1988; Reiter ve ark. 1990). İlk kez Liebowitz ve arkadaşları MAOI'lerinin basit fobi ve atipik depresyonda etkin olduğunu gözlemledikten sonra, sosyal fobi tedavisinde MAOI'lerini denemişlerdir. Yapılan çalışmalarda fenelzinin alprazolam ve plasebodan daha etkin olduğu görülmüştür. Ayrıca reversibl MAO inhibitörü olan moklobemid ve venlafaksin de, sosyal fobi de etkindir (Versiani ve ark. 1992).
Günümüzde tedavide ilk tercih MAOI'leri veya RIMA'dir. Diğer tedavi seçenekleri alprazolam, klonazepam ve SSRI'lardır. Tedaviye ara verildiğinde rölaps olasılığı çok yüksektir. Dolayısıyla farmakoterapi ile başlangıçta sağlanan düzelmenin devam edebilmesi için tedavinin uzun süreli olması ve yanında bilişsel davranışçı tedaviyle desteklenmesi gereklidir.Sosyal fobi ve diğer fobik bozukların uzun süreli tedavisi: ICD-10'da fobik bozukluklar agorafobi, sosyal fobi ve özel fobiler olarak ayrılmıştır. Sosyal fobi çok sıktır, özellikle çocuklukta daha sık görülür. En sık görüldüğü yaş 6 yaştır. Bu tür hastalar için en etkili tedavinin karşılaştırma(eksposure) tedavisi olduğu söylenebilir.Sonuçlar genellikle olumludur ve iyileşme yıllarca devam edebilir. Agorofobide de en etkili tedavi yönteminin davranış tedavisi olduğu söylenebilir. Eğer fobik bozukluğa depresyon eşlik ediyorsa, bu durumda ilaç tedavisi öncelikli olarak düşünülmelidir.Agorafobi, sıklıkla panik ataklarıyla birlikte gider. Hatta agorafobiyi, panik bozukluğun bir varyantı gibi düşünmekte mümkündür. O nedenle bu hastalarda "karşılaştırma" gibi davranış tedavilerinin tek başına uygulanmasından çok kombine tedavilerin uygulanması önerilmektedir. En iyi kombinasyonda imipraminle yapılan kombinasyondur. Yalnızca akut tedavide değil, uzun süreli tedavide de kombinasyonun uygulanması önemlidir. Ancak tedavinin hemen başında kombinasyona geçmek doğru değildir. Öncelikle karşılaştırma, bu yetersiz kaldığında imipraminle kombinasyona geçmek yerinde olur.Sosyal fobi, özellikle geç adolesan dönemde ortaya çıkar. Erkeklerde daha sıktır. Sıklıkla kronikleşir. Eleştirilmekten, reddedilmekten korku duyulur. Beşte bir olguda alkol kötüye kullanımı tabloya eşlik eder. Sosyal fobiyi, kişilik bozukluklarından ve panik haline sekonder olarak gelişen sosyal sakınma davranışından ayırmak önemlidir. Sosyal fobide ilaç tedavisinden daha çok kognitif ve davranış tedavilerinin önemi vardır. Her iki yöntemin kombine edilmesi de mümkündür.İlaç tedavisinin sosyal fobide çok açık bir yararı gösterilememiştir. Bir çalışmada atenololün, bir çalışmada da MAOI'lerinden fenelzinin yararlı olduğu görülmüştür. Alprazolam ve diğer benzodiazepinlerden bağımlılık riski nedeniyle sakınılmalıdır. Eğer ilaç kullanılacaksa bu durumda tedavinin 6 ayı geçmemesi önerilir. Bu süre içinde hasta korktuğu duruma karşı cesaretlendirilir, özgüveni pekiştirilir. Bu durum, hastanın ilaç kesildikten sonra rölaps göstermesine engel olur.PrognozSosyal fobi, yaşamın ikinci on yılında başlar. Bu kadar erken başlangıç ta, çoğunlukla gelişim problemlerine yol açar. Sosyal fobi çoğunlukla sürekli bir gidiş gösterir. İşlevsellikteki bozulmanın derecesi kişinin yaşam şekline, stres faktörlerine göre değişebilir. Sosyal fobi, toplumda sık görülen psikiyatrik bir bozukluk olmasına rağmen, yeterince tanınamamakta ve tedavi edilememektedir. Hastaların tedavi arayışı içine girmemelerinin nedenleri şunlardır:1. Utangaçlıklarının insan doğasının doğal bir yanı olduğunu düşünürler.2. Bu durumun psikiyatrik bir bozukluk olduğunu kabul edemezler.3. Yabancılar karşısında da suskunluklarını devam ettirirler4. Birtakım başetme yöntemleri geliştirmişlerdir.Bütün bunların yanında doktorların da, sosyal fobiyi yeterince tanıyamadıkları da bir gerçektir. Tedavi edilmeyen sosyal fobi, kişinin yaşamında bir çok olumsuz duruma yol açar. Örneğin; okulda başarısızlık, mesleki kısıtlılık, arkadaşlık kuramama, karşı cinsten biriyle birlikte olamama, alkol kötüye kullanımı, depresyon, intihar girişimleri v.b.

Keneden korunmak için en radikal çözüm bunların bulunabileceği ortamlara girmemek; ancak bu çok marjinal bi durum ve pratik değeri de fazla yok. Zira bu “çözüm” doğadan ve onun tüm unsurlarından (çayır, çimen, ağaç, vs..) uzak kalmayı gerektirir ki, bizi çevreleyen cam bir fanusa sahip olmadığımız için uygulama şansı da çok sınırlı. Böylesi etkinliklere en az sempati duyanlarımız bile, hiç olmazsa ayda yılda bir Eymir’e gider, işyerinin bahçesinde öğle yemeğini yer, bahçesinde / balkonunda / terasında mangal yapar, ODTÜ’de çimenlerde oturur, vs...

Kaldı ki, bu mesajın muhataplarının doğa ile iletişimini hiçbir zaman kesmeyeceklerini çok iyi biliyorum; özellikle izci, kampçı ve fotoğrafçı arkadaşların...

Bu çerçevede yaptığım araştırmaların sonuçlarını sizinle paylaşmak isterim.. Bunlar benim ulaştığım ve deneyerek memnun kaldığım sonuçlar; eksik / yanlış ya da eklemek istediğiniz bir konu varsa, açığım elbette...

Öncelikle, bugüne dek başta sivrisinek ve karasinek olmak üzere zararlı haşerete karşı üretilen hemen tüm markalar, son gelişmeleri ardından keneyi de ilgi alanlarına aldılar. Artık ürünlerinin üzerinde “kene dahil her türlü haşereti haklar” gibisinden ifadeler görülüyor. Ben bu olayı, her türden yemek servisi yapan lokantalara benzetiyorum. Hani bir lokantaya gidersiniz, menüde her telden yemek vardır, ama hiçbiri usta işi değildir. Buna karşılık tematik yemek servisi yapan lokantalardan daha fazla memnun kalırsınız, sadece mantı ya da sadece iskender yapan lokantalar gibi, zira adam tek bir işte yoğunlaşmıştır. Kene mücadelesinde en etkili madde “permethrin”. Binde beş oranında permethrin içeren bir karışım keneleri çok kısa bir sürede öldürüyor. Buna karşılık, bu maddenin her miktardaki oranına karşı kenelerde mutlak bir kaçınma eylemi görülüyor; yani eser miktarda bile algılayacak olurlarsa o ortama girmiyorlar. Dolayısıyla, kene kovucu olduğunu iddia eden ürünlerin içeriklerinde bu maddeyi (permethrin) mutlaka arayın ve görürseniz de çekinmeden alın. “Permethrin”, su bazlı ürünlerin içeriğine eklenerek kullanılmakla birlikte, bu ürünler doğrudan insan vücuduna uygulanmıyorlar. Elbise, ayakkabı, çadır, mat, yaygı, uyku tulumu gibi, doğada kullanacağınız eşyaların DIŞ YÜZEYLERİNE ince bir tabaka halinde sıkılıyorlar. Eşyaların yıkanmaması kaydıyla etkisi bir haftadan fazla sürüyor; bu etkin maddeyi algılayan keneler bu eşyalara ve dolayısıyla size ulaşamıyorlar.

Bunların dışında, mesela pikniğe gittiğinizde, ormanda kamp kurduğunuzda, fotoğraf için kamuflajla sotaya yattığınızda, bulunduğunuz yerin çevresine (sınırlı ve ufak bir bölge olduğunu öngörüyorum) kesintisiz olarak bunu sıkarsanız / dökerseniz, kenelerin uçma, hoplama, zıplama gibi atraksiyonları olmadığı için, o çizgiden içeriye girebilmeleri sözkonusu olmaz.

Aynı şekilde, çocuk arabalarının tekerleklerine, piknik masalarının ayaklarına, bahçe / balkon / teras sınırlarınızı oluşturan duvarlara, kapı altlarına, vs.. uygulayabilirsiniz. Tabi kenelerin serbest düşme olanakları olduğu için ağaçların altındayken dikkat edilmesi gerekiyor yine de..

Bu tarz bir ürün olarak Tamay Sağlık ve Zirai Ürünler Şirketinin KEN – KOV isimli ürününü öneririm. Etkisi itibariyle Sağlık Bakanlığı’ndan onaylı ve içeriği itibariyle de Dünya Sağlık Teşkilatı normlarına uygun olan bu ürün 240 gramlık sprey şeklinde satılıyor..

Kullanım konusunu biraz daha açmak gerekirse; doğaya çıkmadan önce, kullanmayı öngördüğünüz giysilerin, eşyaların ve ayakkabıların dış yüzeylerine bu ürün eşit olarak (hafif nemlenmeye başlayana kadar) sıkılır; ancak giysilerin yaka, paça ve diğer açıklıkları ile, çorapların dış yüzeyinin tamamına ve ayakkabıların konç kısımlarına daha yoğun sıkılır. Aynı şekilde, uyku tulumlarının açık kısmına, çadırların kapı kısımlarına, çocuk arabalarının tekerlek çevrelerine, vs... diğer bölgelerine yapıldıklarından daha fazla sıkılır. Bu uygulamayı yapıldıktan sonra giysiler ile eşyalar bir askıda, en az 2 saat (nemli ortamlarda 4 saat) kurumaya bırakılır. Su bazlı olduğu için elbiselerde leke falan bırakmıyor..

DİKKAT: Sadece dış yüzeye sıkılacak (elbiselerin vücuda temas eden iç yüzeylerine uygulanmayacak), etkisini sürdürmesi isteniyorsa elbiseler ve eşyalar yıkanmayacak, yıkandıklarından sonra uygulama tekrarlanacak, ateşe sıkılmayacak, göze bulaştırılmayacak, ÇOCUKLARDAN UZAK TUTULACAK, ağızdan alınmışsa kişinin kusması sağlanacak...

Bunun yanısıra “CHRYSAMED” isimli ürünü de kullanabilirsiniz. KEN – KOV daha çok elbise ve eşya gibi sınırlı alanlara uygulanırken bu ürün daha geniş ortamları kenelerden izole etmek için ideal... Ancak uzman mağazalarda bulabilirsiniz; 5 litrelik bir plastik şişeyi, yanında pompasıyla birlikte, 35-40 YTL. arasında satıyorlar. Belediyelerin kullandığı ürün de ağırlıklı olarak bu.. Eğer doğada sınırları belli bir bölgede uzun süre geçirilecekse (çadırlı kamplar, gün boyu süren piknikler, bahçelerdeki mangal partileri, fotoğraf çekimi için kamuflaj, vs..), ilgili bölgeye pompayla bu üründen sıkılır, sınırları oluşturan çizgiye ise biraz daha fazla sıkılır. Bu tarz bir uygulamayı KEN-KOV’un 240 gramlık spreyi ile yapamayacağınızdan dolayı, bahçenizi, balkonunuzu, terasınızı, ormandaki kamp alanınızı, piknik bölgenizi falan bu ürünle elden geçirebilirsiniz; etkisi 2 hafta kadar sürüyor. Uygulama yaptıktan sonra bir iki saat ilaçlı bölgeyi kullanmayın; sonrasında evcil hayvanlar, çoluk çocuk için zararsız.

Buraya kadar belirttiğim ürünler insan vücuduna uygulanmıyorlar. Eğer tam ve mutlak bir koruma düşünürseniz, bunlardan önce vücudunuza uygulayabileceğiniz bazı ürünler de var. Bu konuda herhangi bir marka belirtmeyeceğim; ancak bu ürünlerin içeriğinde MUTLAKA ama MUTLAKA olması gereken etken madde; “REPELLENT”.
